In standard IVF, 50,000 sperm are placed in a dish with an egg, relying on one "winning" sperm to natural fertilize it. When sperm lack the biology to perform this task, ICSI takes over.
Extremely low sperm counts. When there simply aren't enough sperm to create the enzymatic reaction needed to pierce the egg shell naturally in a standard IVF dish.

ICSI is performed simultaneously with maternal egg retrieval. It requires a highly skilled embryologist operating a multi-axis mechanical micromanipulator under hundreds of times magnification.
The embryologist visually scans thousands of sperm, selecting a single candidate with the most perfect morphology. The sperm's tail is lightly struck to immobilize it.
A gentle holding pipette applies light suction to the egg to keep it perfectly still under the microscope.
A microscopic, hollow glass needle (the injection pipette) containing the single sperm is smoothly pushed through the zona pellucida (outer shell) and into the inner cytoplasm of the egg, where the sperm is released.

ICSI is not inherently "better" but rather a specialized tool. Regular IVF is excellent for tubal issues when the sperm is healthy. ICSI is specifically required when there is severe male factor infertility (low count, poor motility, high DNA fragmentation) or a history of failed fertilization in previous IVF cycles.
